Quick fixes

Black preview or no source. Close other apps using the capture card (OBS, Elgato Studio, other recorders). A capture card can only feed one app at a time.

No screen or window sources. Allow Recaptr in System Settings → Privacy & Security → Screen & System Audio Recording, then reopen Recaptr.

No audio meter moving. Check Microphone access in System Settings → Privacy & Security, and make sure the right input is picked in the sidebar (⌃⌘S).

I hear myself twice. Monitor through headphones, or turn monitoring off. Speakers feed back into the mic.

Where do recordings go?

To the save folder in Settings. With a series set, each take files itself as Series – Ep N in its own folder. Every take with markers gets a matching .fcpxml: double-click it and Final Cut opens the clip with markers in place.

Recaptr quit mid-take

Files are written in a crash-safe way. Open the .mov in QuickTime and you'll have everything up to a few seconds before it stopped.
